I personally like to burn them out. heat up with thin piece of metal like a nail and the Punky wood will burn right out. the solid wood will not burn and it makes it really easy to see what you're working with

Mark, I fill all knots, back or belly with superglue. If there is a void I use fine saw dust and superglue putting it down in layers if deep.

There is no right and wrong. I just personally believe that if it's loose Punky wood it's going to end up popping sooner or later and it might as well be gone. If you end up with a clean hole through the limb you can leave it. But sometimes that knot will go through the limb on an angle and you can see where it goes. When they travel through the limb on an angle you got a great chance for failure later. If it ends up just being a divot you can fill it Like Pat said

Loose knots should probably be removed and if the hole is clean I leave it like that but other than that I do as I said above.

UPDATE
So I worked on this one at Mojam
56" NTN bendy handle lots of knots
I kept hearing a tick while stringing after the bow had been unstrung for a while.
The back broke in the middle of a knot (none of those 1-4)
I had it bending nice at 16"

belly side
this knot

Was the knot where it broke visible from the back? In the picture it kind of looks like it was hidden.

You can see on the back, it lifted from the center of the knot.

Got to thin on the back growth ring at center of knot. :( it's happened to me also. Lesson learned. Sometimes you get away with it sometimes not. Arvin

That sucks buddy! I've seen that happen several times.  Most of the time its the least expected knot that is the problem.  (--)

Remember that tick sound I was talking about.

Yes I do,  I also remember bracing it and couldn't find anything in the back.  Ive done that myself though.
